Wylo Health

Wylo Health is a service dedicated to helping individuals with Weight management, Insomnia and Acne. 

They offer intake questionnaires and online support for clients throughout their journey, and deliver medications (if applicable) to their homes. 

As part of this partnership:

• Patients will be booked with providers and marked as a, "Wylo patient - x condition"
• Providers will review the completed intake questionnaire in the chart to discuss the relevant condition 
• If you feel it is appropriate, you can prescribe a medication for the relevant condition, and fax the prescription to Wylo (fax to 416-391-1957 which will be noted in the chart under, "Alert" section)
• As always, you are providing a medical consultation, not a prescription. Patients select their “Preferred Medication” on the intake form as a note, however this is not mandatory to follow by any means. Non-pharmacologic or other management can certainly be recommended as well! 
• Patients will receive regular check-ins from the Wylo team and be asked to follow-up before further refills if appropriate. 
• If you feel the patient may be appropriate, but you are not comfortable writing a prescription, please use the "GetHelp" button to let PCCs know, and they can assist the patient.
• Visits for patients in Ontario, British Columbia and Alberta are insured and you would bill as in routine visits.
