How will this affect your workflow?
- Euclid will direct patients to Rocket Doctor so they can be seen and assessed by you for a possible referral back to Euclid.
- Our PCCs will ensure you're aware of this as it will be included in the chief complaint in addition to the fax number you'll need to send your referral to.
- You can use our typical referral forms, but if you'd like more information you can see their formal referral form which is attached.
Who are the patients you should be referring to Euclid?
- Eye disease screening for those age 40 or above
- Diabetic patients
- Family History of Eye Disease, OR
- Experiencing ANY of the following:
- Gradual change in vision
- Blurred vision
- Increased difficulty driving at night
