How to create a lab or imaging requisition

To create a lab or imaging requisition through the MD Portal, follow the steps outlined below. This process allows you to generate and send requisitions efficiently, either directly to the patient or to a specified recipient.

Step-by-Step Guide to Generate a Requisition

  1. Start a New Form
    Log into the MD Portal and select the option to create a new form. Choose Lab / Imaging / Test Requisition.
  2. Select the Test and Enter Clinical Information
    Choose the appropriate test from the options provided. If necessary, enter any relevant clinical information.
  3. Send the Requisition
    Once the form is complete, you have several options to send it:
    • Email Copy to Patient: This is the most common option and will send the requisition directly to the patient’s email.
    • Fax to a Specific Number: You can fax the form to a designated fax number.
    • Send to an Alternate Email: If needed, you can enter a different email address to receive the requisition.
    • Fax to a Rocket Doctor Team Member: Select this option if you need to send it to a Rocket Doctor team member for additional processing.

If the Required Form Isn’t Available on the Portal

If you can’t find the specific lab or imaging requisition you need on the MD Portal:

  1. Email Clinical Support
    Send a message to clinicalsupport@rocketdoctor.io with:
    • The clinical indication for the test.
    • Details of the exact test required (e.g., with or without contrast).
  2. Requisition Generation and Review
    Our Clinical Support Team will locate and prepare the necessary requisition. Once ready:
    • Non-Ontario MDs: The requisition will be sent back to you for review and signature.
    • Ontario MDs: A Physician Assistant (PA) can sign on your behalf, eliminating the need for additional action from you.
