Alberta Setup: Provider ID & Submitter ID
Guide to setting up a Provider ID and Submitter ID
To ensure the accurate processing of laboratory requisitions and the timely receipt of results, it is essential to verify your Provider ID and Submitter ID and ensure they are correctly associated with your practice location. Follow this guide to check and update your information as needed.
Understanding Provider ID and Submitter ID in Alberta
Provider ID
- A unique identifier assigned to each practitioner in Alberta.
- Stays the same regardless of where you practice.
- Used on all requisitions to identify you as the ordering provider.
Submitter ID
- Identifies the clinic location where laboratory tests are ordered from and results are routed to.
- If you practice in multiple locations, you may have multiple Submitter IDs for different sites.
- When working with Rocket Doctor, you act as your own clinic and need a Submitter ID specifically for Rocket Doctor.
Step 1: Verify Your Provider ID and Submitter ID Information
- Go to Alberta Provider ID Lookup.
- Log in using your Name and PRACID.
- Find Your Provider ID.
- Search for your Rocket Doctor Submitter ID and verify the details.
Your Submitter ID should be listed as:
📝 Dr. [First Name] [Last Name] ROCKET DOCTOR INC.
Ensure the following details are correct:
✅ Practice Location:
- Must be an address where you are comfortable having lab results sent if fax or phone contact fails.
- It should NOT be Rocket Doctor HQ in Toronto.
- Patients do not see this address, but lab technicians and healthcare professionals with a PRACID can view it.
- If you prefer to keep the address private, add the note: (HOME ADDRESS).
✅ Fax Number: Your Rocket Doctor eFax number.
✅ Phone Number: A number you are comfortable receiving critical results on.
Step 2: Request or Update Your Submitter ID
If your Submitter ID information is missing or needs updating, follow these steps:
- Complete the Request for Provider/Prescriber Set-Up in Health Information Systems form
- Select the appropriate request type:
- New LOCATION (if requesting a new Submitter ID).
- Update Location Demographics (if updating an existing Submitter ID).
- Ensure your Practice Location follows the same guidelines as in Step 1:
- Do NOT use Rocket Doctor HQ in Toronto.
- If preferred, add (HOME ADDRESS) for privacy.
- Connect Care Submitter ID:
- Leave blank if setting up a new one.
- Clinic/Location/Client/Office Name:
- Rocket Doctor
- Result Report Delivery Method:
- Check "Fax."
- Complete the Provider/Prescriber Information section.
- Submit the completed form via email to:

Step 3: Complete Your Provider Declaration
- Fill out the Virtual Health Application with the following details:
- Provider Declaration: Rocket Doctor
- Rocket Doctor Address (Use an appropriate address, NOT HQ in Toronto).
- Provider Business Address
- Sign the form
- Submit the form
Step 4: Update Your Provider ID and Submitter ID in the Rocket Doctor Portal
- Log in to Starship:
🔗 Rocket Doctor Portal - Go to:
- Account Settings → License/Billing Numbers
- Update your Provider ID and Submitter ID.
- Click "Save."
- Mark the onboarding task as completed! ✅
