Guide to Setting Up Lab Result Routing for Rocket Doctor

We understand the importance of receiving laboratory and imaging results directly to your e-fax number. To facilitate this, we have created a step-by-step guide to help you set up your account with common labs in Ontario.

Important Notes


Address Requirement

Rocket Doctor is not a clinic, and its corporate address must not be used for lab setup. Instead, if you do not have an alternate clinical address, consider using a mail-forwarding service or a PO Box. Software platform addresses (e.g., OSCAR, Telus PSS, Accuro) cannot be used as your clinic address.



1. LifeLabs 

To have your results faxed directly to your RD e-fax number via RightFax, complete the following:

1. Fill Out the Online Form
Healthcare Provider Information Request (2025)

Complete these fields:

  • Purpose of Healthcare Provider Information Update: NEW Request
  • Province: Select Ontario
  • Report Delivery: Choose Fax (⚠️ do NOT select EMR)
  • Fax Number: Enter your Rocket Doctor fax number
  • Current Clinic/Practice/Hospital Address: Use your own office, PO Box, clinic, or home address
    (Important: do not list Rocket Doctor — we do not have a mailing address or staff monitoring mail)
  • Phone Number: Provide a direct line for urgent results
  • Backup Physician: You can list yourself

2. Submit the Form
Submit online (no faxing needed).

3. Notify Provider Success
Please email Provider Success once you’ve submitted the form so we can track your setup.

4. Verification Form

  • Within 5 business days, LifeLabs will send a fax verification form to your Rocket Doctor fax number.
  • Please watch for this fax, sign it, and return it promptly.
  • If you’d like Provider Success to sign on your behalf, include your consent when you email the team in Step 3.
  • ⚠️Ontario-specific: If your fax is not yet verified, results will be mailed to the practice address listed on your requisition until verification is complete. Once verified, results will be faxed to your Rocket Doctor number.

2. LifeLabs Launchpad (Optional)

Launchpad provides online access to patient bloodwork completed at LifeLabs locations.

Step-by-Step:

  • 📄 Complete the Form: Launchpad Online Enrolment Form (PDF)
    • Use your personal address (not 180 John street) and contact info on the form.
  • Send your completed form by:
    • 📧 Email: ITServiceDesk@lifelabs.com
    • 📠 Fax: 1-888-667-4909

Application processing time is approximately 10 business days.


3. Dynacare 

Email Dynacare to request automatic e-faxing of lab results:

Step-by-Step:

  • 📧 Email: physicianrelations@dynacare.ca to request automatic e-faxing of results
    • Subject Line: Rocket Doctor Update
    • Include the following in your message:
      • CPSO Number
      • OHIP Billing Number
      • Personal Phone Number
      • Practice Address (not 180 John street)
      • RD e-fax number
    • Report Setup Instructions:
      • Turn ON auto fax
      • Turn OFF paper reports
      • Slot Code: JTO

After-Hours Contact Info (for results):
Between 8:00–23:00: Call 647-697-6973 to reach the doctor on call.


4. Public Health Ontario (PHO)

Step-by-Step:

  • 📧 Email: customerservicecentre@oahpp.ca to request all test results be faxed to your RD e-fax
    • Include your:
      • CPSO number
      • Billing number
      • Practice address (not 180 John street)

📝 Sample message to include:

Please have all results I order from my virtual practice to be faxed to the following PIPEDA/HIPAA-compliant fax number: [Your RD Fax Number]. Please send me the fax waiver form or confirm your setup process to ensure I receive my results by fax.
